9 months of age TB Screening, Lead Test, Complete Blood Count
12 months of age MMR, Varivax, (+/-)PCV
15 months of age DTaP, Hib, IPV, (+/-)PCV
18 months of age Hepatitis A
2 years of age Hepatitis A
4 years of age DTaP, IPV, MMR, Varivax, PPD, Hearing, Vision
11-12 years of age Tdap, Menactra, HPV (females)
All other ages Annual exam with immunization boosters as needed
Influenza Vaccinations are recommended annually for children 6 months or older.
Hepatitis B- First dose is given at birth
Hepatitis A- May be given as early as 12 months
DTaP- Diphtheria, tetanus, pertussis Gardasil- Human Papilloma virus
Hib- Haemophilus influenza Menactra- Meningococcal
IPV- Polio Vaccine Tdap- Tetanus, Diphtheria, Pertussis
Rotateq- Rotovirus PPD- Skin Test for Tuberculosis
PCV- Pneumococcal Vaccine
